- _{The Member will be considered a new patient if he/she has not visited any Eu Yan Sang clinic for 24 months or more.}
- _{Valid Parkway Plus e-card from MyHealth360 App must be presented upon registration.}
- _{Valid for in-clinic consultation all Eu Yan Sang TCM Clinics, except at Eu Yan Sang Premier TCM Centres, Eu Yan Sang TCM Centre and Sessional Clinics.}
- _{Full consultation charge applies if no treatment or medication is taken.}
- _{Valid with all Physicians.}
- _{Limited to one redemption per customer.}
- _{Not applicable for patients who are not medically suited to take herbal medication or other treatments.}
