_{The recommended patient must provide the recommender’s name and contact number during registration. Rewards will only be awarded if the provided name and mobile number match our clinic records.}
_{The recommended patient must be a new patient to Eu Yan Sang TCM Clinics, rewards are awarded after the first transaction.}
_{The patient will be considered a new patient if he/she has not visited any Eu Yan Sang clinic for 24 months or more.}
_{Valid for in-clinic visits and TCM services only, excluding retail products.}
_{Offer is only valid with the purchase of medication or administration of treatment.}

_{Voucher is non-refundable and non-exchangeable for cash. If the value of the voucher is not fully redeemed, no refund in cash or in other means will be given.}
