_{Offer is not valid at Eu Yan Sang Premier TCM Centres, Eu Yan Sang TCM Centre and sessional clinics.}
_{Offer is not valid with Senior Physicians.}
_{Offer is applicable to Physicians, a top-up of S$5 for Associate Physicians is required.}
